Graduation Story:
After eight months in our program, Dana has graduated by obtaining insurance through her employer. Dana came to the program referred by her Primary Care Physician (PCP) after a mass was found through a routine mammogram screening. The patient was screened for eligibility and enrolled in our program. Through specialty referrals obtained from her PCP the patient was able to obtain a biopsy on the mass. The biopsy indicated she was positive for Invasive Carcinoma and Ductal Carcinoma in Situ and would need a lumpectomy. Dana was required to have an echocardiogram to determine if she was able to undergo the procedure. Once cleared by the echocardiogram, she had surgery to remove the mass. She was then referred to a cancer center for 30 days of radiation therapy. Dana is now cancer free and has now been released from care through the cancer center. She has been able to go back to work full time and has obtained medical insurance benefits as a result. She has been extremely grateful for the prompt and caring attention she received from the Care Coordinators while in the program. She was also appreciative of the Breast Health information she received as part of the education provided by the Care Coordinators. |
Programs NorthlandCARE, a volunteer network of health care providers donating primary care for uninsured and underinsured residents. NorthlandCARE’s referral system ensures that providers are asked to accept only the number of patients they have agreed to see. Referrals rotate among the providers to maintain a balanced distribution of patients. Blue Cross Blue Shield of Greater Kansas City partners with NorthlandCARE in providing a non-payable electronic claims system that assesses medical costs for provided patient services. The Healthy Pregnancies Program, in partnership with Samuel U Rodgers, provides Care Coordination to patients through this new and innovative program.
